29 CFR 1910.147(c)(7)(i):The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: a) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - Untrained mechanics were exposed to amputation hazards while working on a timing issue on an energized Gyoza Number 4, Daehan Machinery Co. Ltd. Model DHGZ-22, Serial Number 190218-10004 dumpling machine. Violation was observed on or about December 13, 2021. b) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - An untrained mechanic was exposed to amputation hazards while working on a maintenance issue on an energized Wonton machine, Daehan Machinery Co. Ltd. Model DHW-32. Violation was observed on or about December 13, 2021. c) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - The untrained Engineering Manager was exposed to caught in hazards while working on a maintenance issue with the W.E. Plemons Machinery Services, Inc. Model # 3000800 Serial # 18-09-01809 Boxing Machine as he did not have the machine locked out or tagged out and an unknowing employee started the machine. Violation was observed on or about January 20, 2022. d) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - Untrained Mechanics were exposed to amputation hazards while working on a maintenance issue as they had a lock on the machine that was not placed correctly, the key was left in the lock on the W.E. Plemons Machinery Services, Inc. Model # 3000800 Serial # 18-09-01809 Boxing Machine. Violation was observed on or about February 16, 2022. e) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - Untrained Mechanics were exposed to amputation hazards while working on replacing a door interlock switch as they hung an "Equipment Out Of Service" tag but did not lock the machine out after shutting it off on the Ilapak Packaging Machinery Serial Number 0642220241 packaging machine. Violation was observed on or about February 17, 2022. f) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - A untrained mechanic was exposed to amputation hazards while working on clearing an obstruction as he did not shut off the main power on the Daehyun Hi-Tec. Co. LTD, Model Number DH33ED, Serial Number 2K11712144. Violation was observed on or about February 18, 2022. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
