29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: The employer is failing to protect employees from struck-by, crushed-by, and electrical hazards associated with trenching. This was most recently documented on October 14, 2021, at the residential construction site located at the corner of SW Valley Drive and SW Meadowood Drive, Grain Valley, Missouri. An employee was working in and around an excavation. The employer did not instruct the employee in the recognition and avoidance of unsafe conditions associated with excavation related work and the regulations applicable to excavation-related work. Arrow Plumbing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.21(b)(2), which was contained in OSHA inspection number 1197640, citation number 2, item number 1 and was affirmed as a final order on November 15, 2018, with respect to a workplace located at 507 Coleman Road, Belton, Missouri.
