29 CFR 1918.97(e)(1):The employer did not ensure that there was in the vicinity of each vessel being worked, at least one U.S. Coast Guard approved 30-inch life-ring with no less than 90 feet of line attached, and at least one portable or permanent ladder that reaches from the top of the apron to the surface of the water: (a) on or about 9/22/2021 and at times prior thereto, employees working to unload the f/v Dominic II and the f/v Silver Point were exposed to hypothermia hazards, in that the pier and dock on which they were working did not have a ladder installed or available to use for egress from the water in case an employee fell in the water.
