29 CFR 1910.213(r)(4): Suitable guard(s) were not provided to reduce to a minimum the hazard due to the point of operation of knot saws: On or about July 20, 2021 and at times prior to, at the worksite located at or near 190 Wilson Mill Road in Monticello, Arkansas, employee(s) were exposed to amputation hazards when operating optimizing knot saws. The MAXWELL HARDWOOD FLOORING, INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.213(r)(4), which was contained in OSHA inspection number 1235423, citation number 1, item number 6 and was affirmed as a final order on 11/06/2017, with respect to a workplace located at 190 Wilson Mill Road in Monticello, AR.
