29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: On or about May 13, 2021, for the establishment, within 8 hours after the deaths of two employees as a result of a work related incident, the employer did not report the fatalities to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. Pursuant to 29 CFR 1903.19, within thirty (30) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure the employer reports within 8-hours the death of an employee from a work-related incident to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor.
