29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) Western Bee Supplies Inc.: On August 6, 2020, an employee of Western Bee Supplies Inc. suffered a finger amputation when contacting a rotating blade on the Paulson Enterprises, Inc. Bottom Gang Rip Saw Model SL-10 Serial #63. The employer failed to report the amputation within 24 hours to OSHA. The amputation was reported on August 11, 2020.
