29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). (Construction reference 29 CFR 1926.602(d)): BB Frame LLC dba Frame Q LLC, as successor to Frame Q LLC, and Juan Quevedo does not ensure that employees operating All Terrain Forklifts are trained and evaluated. This violation was most recently observed at 2400 6th St, Fort Lee NJ, at instance (a): Exterior of Building - Front - where employees were exposed to struck by hazards while an All Terrain Forklift was being operated by am employee that had not been trained and evaluated on the safe operation of forklifts, on or about 12/31/19. To abate this violation the employer must ensure that its employees who operate a forklift are trained and evaluated in the safe operation of forklifts. Frame Q LLC was previously cited SERIOUS for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.602(d), which was contained in OSHA inspection number 979064, citation number 01, item number 03 and was affirmed as a final order on 10/25/2017, with respect to a workplace located at 18 Skyline Dr, Englewood Cliffs, NJ 07632.
