29 CFR 1926.403(b)(2): Installation and use. Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: location: 805 Mamaroneck Ave., Mamaroneck, NY 10543, exterior of building on or about: 4/9/19 a) A quad receptacle was not installed in accordance with its listing. It was attached to the end of an extension cord and it was not mounted. The receptacle was used by an employee to power a Chicago Electric spade handle drill, while engaged in exterior insulation and finish system (EIFS) work. NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.
