29 CFR 1904.29(b)(1): 29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation. Located at 16 Avenue E, Hopkinton, MA On or about October 3, 2018, the following recordable injuries were not adequately described on the OSHA 300 log for calendar year 2018; Case #18 - The employer recorded that the employee cut a finger, however did not provide an adequate description as to what caused the cut to the finger. Case #19 - The employer recorded that the employee partially amputated a finger, however did not provide an adequate description as to what caused the partially amputated finger.
