29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) Sewer Solutions located at 1374 Yates Street, Denver, CO 80204: On and Preceding September 20th, 2018, the employer did not ensure that a work-related incident resulting in in-patient hospitalization was reported within 24 hours. Workers were digging and working in a trench which was up to approximately 12 feet deep to tie in to a main sewer line for a new construction multi-family dwelling. A trench collapse resulted in employee injuries requiring in patient hospitalization on 9/20/2018 at approximately 1:15 PM. The employer did not report the hospitalization until 24 September, 2018 at 2:54 PM.
