29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Production/Dough Room Line D Depositor A: Procedures for the dough depositor were not utilized when the depositor was cleaned by sanitation. On 5/13/18 an employee suffered an amputation of the right middle finger while cleaning the hopper on the depositor, on or about 5/14/18. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19. J&J Snack Foods Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910. 147(c)(4)(i), which was contained in OSHA inspection number 1075323, citation number 1, item number 1a and was affirmed as a final order on 1/11/2016 with respect to a workplace located at 361 Benigo Blvd, Bellmawr, NJ 08031.
