NOTE: A sign reading DANGER -- PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign. On or about 04/18/2018, at CITGO Refinery West Plant facility located in Corpus Christi Texas, the employer did not inform employees by posing danger signs or by any other equally effective means on workplace contains permit spaces for the following but not limited to. a. Aromatic Splitter (546-V9) skirt in #5 Platformer unit b. Depropanizer (546-V032) skirt in #5 Platformer unit c. Debutanizer (546-V034) skirt in #5 Platformer unit d. Fractionator (527-V9) skirt in Mid Distillate Hydro-treater (MDH) unit
