29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a. On or about March 8, 2018, employees used a Vestil, WP-3737-FD, man basket attachment on the forks of a Yale, Model GLC030VXNURE082, Serial C809V05199H, propane powered forklift to change out light bulbs. The employer did not ensure that the attachment was approved by the forklift's manufacturer prior to use.
