29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: 101 North Main Street, Newark, Illinois, west building shop, powered industrial vehicles - on or about May 4, 2017, employees operating forklifts for material handling and dump trailers towing were not evaluated in safe operation of the equipment as specified in the standard at least once every three years. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). Louis Dierzen dba Dierzen Sales, Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(4)(iii) which was contained in OSHA inspection number 1125876, citation number 03, item number 001 and was affirmed as a final order on August 29, 2016; and, OSHA Inspection number 957315, citation number 02, item number 001 and was affirmed as a final order on June 20, 2014, with respect to a workplace located at 101 N. Main St., Newark, IL 60541.
