29 CFR 1904.32(b)(5): The Summary of Work-Related Injuries and Illnesses (OSHA Form 300A or equivalent) for the previous year was not posted at each establishment in a conspicuous place or places where notices to employees are customarily posted. The posted annual summary shall not be altered, defaced or covered by other material: (a) 1511 Washington Street East: On or about March 8, 2017 and times prior, the employer did not post the OSHA 300A, Summary of work-related injuries and Illnesses at the site or at the shop located in Nitro, WV. Tri-State Pipeline, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1904.32(b)(6); Annual summary posting, which was contained in OSHA inspection number 409703, citation number 2, item number 1 and was affirmed as a final order on September 11, 2012 with respect to a workplace located at the jobsite in Crab Orchard, WV.
