29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a. On or about February 7, 2017, located East of the hub cutters, near the elevator lift and normalizing furnace, employees used an approximately five foot long single pole attachment on a Hyster, Model H60XM, Serial Number H177B51078B, forklift to transport wheels without the data plate being updated to include the capacity of the forklift with the pole attachment installed, thereby exposing employees to a struck-by hazard.
