29 CFR 1904.39(a)(1): The employer did not, within eight (8) hours after the death of any employee as a result of a work-related incident, report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor: a) Job site, 406 Rose Drive, Cranberry Township, PA: On or about December 1, 2016, the employer failed to report within 8 hours the death of an employee that resulted from a fall from the roof of a 2 story home.
