29 CFR 1910.1030(f)(2)(iv): The employer did not assure that employees who decline to accept hepatitis B vaccination offered by the employer sign the statement in Appendix A. a. Haviland Plastic Products Inc. located in Haviland Ohio: On or about November 16, 2106, the employer did not ensure an employee responsible for cleaning up blood after an industrial accident near machine #20 signed a declination form when he declined the hepatitis B vaccination series.
