29 CFR 1910.184(e)(4): Certificate(s) of proof test were not available for new, repaired or reconditioned alloy steel chain slings: The employer does not proof test alloy steel chains before use at the facility. This violation occurred on or about July 19, 2016, and time prior to at the facility, exposing employees to the hazard of being struck/crushed by falling loads. Contract Manufacturer dba CM Trailers was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.184(e)(4), which was contained in OSHA inspection number 316238658, citation number 1, item number 5, and was affirmed as a final order on October 17, 2012, with a final abatement date of September 28, 2012, with respect to a workplace hazard at 1102 N. Industrial Road Madill, OK 73446. Contract Manufacturer dba CM Truck Beds was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.184(e)(4), which was contained in OSHA inspection number 316238732, citation number 1, item number 1, and was affirmed as a final order on October 17, 2012, with a final abatement date of September 28, 2012, with respect to a workplace hazard at 1102 N. Industrial Road Madill, OK 73446. Pursuant to 29 CFR 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure that all alloy steel chains are proof tested.
