29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of a powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) In the trailer production facility: On or about May 18, 2016, and at times prior thereto, employees were exposed to hazards, in that the employer used extended forks on a Hyster #33 forklift without prior written approval from the manufacturer.
