29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: 101 North Main Street, Newark, Illinois, west building shop, powered industrial vehicles - on or about February 18, 2016, employees operating forklifts for material handling and dump trailers towing were not evaluated for competency in safe operation as specified in the standard at least once every three years. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). The Dierzen Trailer Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(4)(iii), which was contained in OSHA inspection number 957315, citation number 2, item number 1 and was affirmed as a final order on June 20, 2014, with respect to a workplace located at 101 N. Main St., Newark, IL 60541.
