29 CFR 1904.33(a): The employer did not save the OSHA 300 Log, the privacy case list (if one existed), the annual summary, and the OSHA 301 Incident Report Forms for five (5) years following the end of the calendar year that these records covered. (a) 4197 E Harrisburg Pike, Middletown Pennsylvania - The employer did not maintain and retain the OSHA 300 log and OSHA 300A summary of work place injuries and illnesses for 5 years following the end of the calendar year, on or about September 30, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
