29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) 516 Main Street, Blandon, PA; Employees were installing a 3/4 inch water line while working in a 5 feet 8 inch deep excavation without a stairway, ladder, ramp or other safe means of egress located within 25 feet of the employees, on or about 09/15/2015. M&A Excavating, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.651(c)(2), which was contained in OSHA inspection number 314724923, citation number 1, item number 1 and was affirmed as a final order on 09/21/2011, with respect to a workplace located at Glover Road, Easton, PA 18040. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
