29 CFR 1904.7(b)(2): Employer must record an injury or illness that results in death by entering a check mark on the OSHA 300 Log in the space for cases resulting in death. On or About July 15, 2015 R/J Group, Inc owners submitted OSHA 300 and 301 logs for years 2013, 2014, and 2015 to date. Employer did not properly record the fatal injuries of a R/J Group Inc employee on the 2015 OSHA 300 logs.
