29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) On or about the week of March 1, 2015, in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not utilized by the machine operator during setup and dies changes on the 750 ton HPM hydraulic press. Forging Press LO/TO machine specific procedures exists and operators are trained as authorized employees but neither are periodically inspected by the employer. b) On or about the week of April 8, 2015, in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not utilized by the machine operator during die changes on the 3 inch National Upsetter. Forging Press LO/TO machine specific procedures exists and operators are trained as authorized employees but neither are periodically inspected by the employer. c) On or about April 9, 2015, in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not fully utilized by the machine operator during die changes on the 150 ton Maxi Press. Forging Press LO/TO machine specific procedures exists and operators are trained as authorized employees but neither are periodically inspected by the employer. d) On or about April 9, 2015, in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not utilized by the saw operator during a blade change on the Horizontal Cantilever Band Saw (end). Horizontal Bandsaw (Spartan-Saw) LO/TO machine specific procedures exists and operators are trained as authorized employees but neither are periodically inspected by the employer. e) On or about April 9, 2015, in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not utilized by the saw operator during a blade change on the Horizontal Cantilever Band Saw (middle). Horizontal Bandsaw (Spartan-Saw) LO/TO machine specific procedures exists and operators are trained as authorized employees but neither are periodically inspected by the employer. The Dyson Corporation was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 315443630, citation number 1, item number 2 and was affirmed as a final order on 10/27/2011, with respect to a workplace located at 53 Freedom Rd., Painesville, Ohio 44077.
