Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards accociated with the lack of adequate training and instruction in the recognition and avoidance of unsafe conditions associated with the erection, maintenance and disassembly of various scaffolding. Among other methods one feasible and acceptable abatement method to correct this hazard is thoroughly to train and instruct all stage builders in the particular hazards associated with the erection, maintenance and disassembly of various scaffolding: Throughout the Worksite - Stagebuilders were not thoroughly trained and instructed in the particular hazards associated with the erection, maintenance and disassembly of various scaffolding including tube and coupler, kwikstage and superscaf in that adequate visual training aids were not provided and specific instructions were not given in regards to the use of planking during the erection, maintenance and disassembly of those types of scaffolding. Also, detailed information provided by the manufacturer on the kwikstage scaffolding system was not included in the Quality Assurance Instruction Job Safety Procedure Temporary Scaffolding, Ser #QA1-01-2-71. Your company was previously cited for a violation of this Occupational Safety and Health Standard which was contained in OSHA inspection No. 1014503386, Citation #1, item 1, issued on 11/04/87.
