On or about October 4-25, 2013, at the Harlem Valley Psychiatric Center worksite, employees disturbed, removed, and swept up debris from asbestos-containing tile, carpet with asbestos-containing mastic, and thermal systems insulation, inside buildings including but not limited to Buildings 3, 11, 12, 14, 17, 29, 30, 31, 32, 35, 85, and 107.
