Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: On or before June 1, 1990 to on or about July 16, 1990, employees operating a freight elevator, and/or riding a freight elevator, and/or working in the vicinity of a freight elevator were exposed to the hazard of falling into the elevator shaft and/or being crushed by the elevator cab. a) Location: Elevator #2, Floor 5, Door - B Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1) Equip each hoistway door with a hoistway door interlock device. 2) The interlock device shall hold the door in the locked position by means of gravity or by a restrained compression spring, or by both, or by means of a positive linkage. 3) The interlock shall lock the door in the closed position before the elevator can be operated.
