29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a) ADA Dash Subline - A06-1/A06-2; On or about June 18, 2016 and at times prior thereto, Employee #2 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter the robotic cell with energized robots and other jig and fixtures to replace a welding tip on a robot. The employee was unable to lock out the robotic cell because the employer had not provided or ensured the employee had a lock. b) JFA Quarter Line L/H ? A11/A12/A15; On or about June 17, 2016 and at times prior thereto, Employee #18 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter the robotic cell with energized robots and other jig and fixtures to clean. The employee was unable to lock out the robotic cell because the employer had not provided or ensured the employee had a lock. c) ADA Dash Main Line - A900; On or about June 18, 2016 and at times prior thereto, Employee #9 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter the robotic cell with energized robots and other jig and fixtures to replace a welding tip on a robot. The employee was unable to lock out the robotic cell because the employer had not provided or ensured the employee had a lock. d) ADA Dash Main Line - A910; On or about June 17, 2016 and at times prior thereto, Employee #36 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter the robotic cell with energized robots and other jig and fixtures to replace a welding tip on a robot. The employee was unable to lock out the robotic cell because the employer had not provided or ensured the employee had a lock. e) ADA Dash Main Line - A900 and JFA Quarter Line L/H ? A01/A02/A03; On or about June 17, 2016 and at times prior thereto, Employee #30 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter the robotic cell with energized robots and other jig and fixtures to replace a welding tip on a robot. The employee was unable to lock out the robotic cell because the employer had not provided or ensured the employee had a lock. f) JFA Quarter Line R/H and ADA Dash Subline; On or about June 17, 2016 and at times prior thereto, Employee #20 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter the robotic cell with energized robots and other jig and fixtures to replace a welding tip on a robot. The employee was unable to lock out the robotic cell because the employer had not provided or ensured the employee had a lock. g) ADA Dash Subline A20 RH/A10 RH/S11 RH; On or about June 17, 2016 and at times prior thereto, Employee #12 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter the robotic cell with energized robots and other jig and fixtures to replace a welding tip on a robot. The employee was unable to lock out the robotic cell because the employer had not provided or ensured the employee had a lock. h) JFA Quarter Line L/H ? A06/A101/A05 and ADA Dash Main Line- A700; On or about June 17, 2016 and at times prior thereto, Employee #28 was exposed to caught-in, struck-by, and crush hazards in that employee was required to enter the robotic cell with energized robots and other jig and fixtures to replace a welding tip on a robot. The employee was unable to lock out the robotic cell because the employer had not provided or ensured the employee had a lock. l) ADA Dash Subline A20 LH/A10 LH/S11 LH; On or about June 17, 2016 and at times prior thereto, Employee #3 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter the robotic cell with energized robots and other jig and fixtures to replace a welding tip on a robot. The employee was unable to lock out the robotic cell because the employer had not provided or ensured the employee had a lock. j) ADA Dash Main Line- A900; On or about June 17, 2016 and at times prior thereto, Employee #15 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter the robotic cell with energized robots and other jig and fixtures to replace a welding tip on a robot.
