29 CFR 1910.28(b)(6)(ii): The employer did not ensure that each employee 4 feet (1.2 m) or more above dangerous equipment must be protected from falling by one of the systems listed in 29 CFR 1910.28(b)(6)(ii)(A)-(D): a) The employer fails to ensure effective measures to prevent fall hazards of up to eight feet onto dangerous equipment as demonstrated on or about August 23, 2023, in the Sawmill when crossing over the outside platform guardrails to access the incline chip auger and when standing on the structural steel beams over the conveyor of the shaker table, where an employee works to clear jams. To abate this hazard, the employer must ensure employees are protected from falling from the platform onto the conveyor when outside of the guardrails and from falling onto the shaker table while on the structural steel beams to access the chipper vent.
