29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a. On or about 12/12/2019, at the fabrication area near the small MEP bending machine, employees were exposed to electrical hazards, in that, the wiring to the fan had a prong that was missing the ground prong. b. On or about 12/12/2019, at the fabrication area employees were exposed to electrical hazards, in that, the wiring to the welder had a prong that was missing the ground prong.
