29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s): Block Plant, Mixer: On, or about, June 18, 2019, an employee(s) who entered the mixer at the conclusion of each shift was not assigned a lockout device to isolate the disconnect switch. He normally used an unlabelled lock that was stored on the disconnect device.
