29 CFR 1910.253(a)(3): Apparatus such as torches, regulators or pressure-reducing valves, acetylene generators, and manifolds used in an oxygen-fuel gas system had not been approved: a. On or about 12/12/2019, at the fabrication area, employees were exposed to a fire hazard, in that, the oxygen-acetylene manifold was not equipped with an approved gauge. The acetylene gauge used was broken and not replaced.
