29 CFR 1926.52(d)(1): In all cases where the sound levels exceeded the values shown in Table D-2 of 29 CFR 1926.52, a continuing, effective hearing conservation program was not administered: The employer did not administer an effective hearing conservation program, inclusive of noise monitoring, training, baseline and annual audiograms, for employees who were exposed to sound levels that exceeded the values shown in Table D-2 of 29 CFR 1926.52, such as, but not limited to, the following: a) On June 17, 2015, a grounds man/cleaner was exposed to a noise exposure dose of 624.7%, or 103.2 dBA Time Weighted Average. The sampling was conducted over 427 minutes. b) On June 17, 2015, a crew leader was exposed to a noise exposure dose of 148.3%, or 92.8 dBA Time Weighted Average. The sampling was conducted over 421 minutes.
