29 CFR 1910.134(f)(2):The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. At the facility located at 1602 N 8th Street, Manitowoc, WI 54220, the employer did not ensure that each employee required to wear a tight-fitting half-face respirator while performing work with silica sand containing up to 90.0% silica (quartz, total) passed a qualitative fit test (QLFT) or quantitative fit test (QNFT) annually, exposing each employee to the hazards associated with respirable crystalline silica. The employer required employees to wear filtering facepiece respirators to protect employees from respirable crystalline silica. Eck Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(f)(2), which was contained in OSHA inspection number 1304681, citation number 1, item number 1b and was affirmed as a final order on May 22, 2018, with respect to a workplace located at 1602 North 8th Street, Manitowoc, WI 54220.
