29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: On or about August 5, 2019: a. Miller welder in Fab 1, cord was repaired with electric tape. b. The foot pedal of the Geka Iron-worker Hydracrop 110/A in Fab 2 was damaged and lying in a puddle of water. c. Extension cord providing power to the fan on the southwest floor of Fab 2 was repaired with electric tape.
