29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s). a). Yard Area - On or about and times prior to March 2, 2016, the #4 Ottawa Commando 30 yard tractor was operated with a cracked windshield, one non-functional headlight, and a non-functional audible reverse alarm, thus exposing employees to struck-by hazards. b). Yard Area - On or about and times prior to March 2, 2016, the #5 Ottawa Commando 30 yard tractor was operated with a cracked windshield, one non-functional headlight and a non-functional audible reverse alarm, thus exposing employees to struck-by hazards. c). Yard Area - On or about and times prior to March 2, 2016, the #205 Ottawa Commando 30 yard tractor was operated with one non-functional headlight and cracked side mirrors, thus exposing employees to struck-by hazards. Great Southern Peanut, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(p)(1) which was contained in OSHA inspection number 962546, citation number 1, item number 17b and was affirmed as a final order on January 9, 2015, with respect to a workplace located at 132 Fifth Street, Leesburg, GA 31763. Georgia Farm Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(p)(1) which was contained in OSHA inspection number 942122, citation number 1, item number 12c and was affirmed as a final order on January 9, 2015, with respect to a workplace located at 132 Fifth Street, Leesburg, GA 31763.
