29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. On or about 14 February 2018, the employer failed to install and use equipment in accordance with the instructions listed and permitted uses. Industrial Start/Stop switches were replaced with standard, household-use light switches on the following machines: a. National Bolt Maker #19, at the #40 conveyor system. b. National Bolt Maker #17, at the conveyor. This exposed employees to burns from arcing faults and electrical fires. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
