29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided: On or about 14 February 2018, the employer failed to properly enclose all sprockets and chains with an adequate enclosure to ensure that employees did not inadvertently come into contact with the amputation hazards on the following machines: a. National Bolt Maker #19: The chains and sprockets on the conveyor belt system were not enclosed. b. National Bolt Maker #19: The chains and sprockets on the conveyor belt system #40 were not enclosed. c. National Bolt Maker #17: The access door on the side of the machine was tied open, which exposed employees to chains and sprockets that were not enclosed. d. National Bolt Maker #17: The chains and sprockets on the conveyor belt system was not guarded. This exposed employees to amputation hazards associated with rotating parts when required to perform maintenance, set-up, or when working at machine(s). Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
