29 CFR 1910.219(b)(1): Flywheels. Flywheels located so that any part is seven (7) feet or less above floor or platform shall be guarded in accordance with the requirements of this subparagraph: On or about 14 February 2018, the employer failed to ensure that the proper guarding was in place to prevent employees from inadvertently contacting rotating flywheels during the operating cycle of machines. The flywheels on the following machines were unguarded or had exposed portions that created caught-in and struck-by hazards: a. National Bolt Maker #19. The lower portion of the flywheel did not have any form of guarding to prevent employees from inadvertently contacting spinning spokes on the flywheel during operation. b. National Bolt Maker #17. The access doors to the flywheel were tied open, which exposed employees to inadvertent contact with spinning spokes on the flywheel during operation. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
