29 CFR 1915.152(c): Defective or damaged personal protective equipment was used. On or about December 12, 2024, and at times prior thereto, employee(s) used defective or damaged personal equipment to conduct welding and grinding operations in support of U.S Navy targets, in the following locations: a) Bldg. 1396, Right hand Miller TIG gloves was damaged on the right thumb. The damaged area was wrapped with green tape. b) Bldg. 1396, Miller welding jacket Part #231082 was damaged with multiple holds throughout the jacket. c) Bldg. 1405, Miller welding jacket Part #258100 was damaged with multiple holes throughout the jacket and had one broken garment button. Employee(s) were potentially exposed to fire, UV radiation and heat hazard(s).
