29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a. On or about 12/12/2019, at the yard area where forklift operations were conducted, employees were exposed to the hazard of operating a Ring forklift with extension attachments to the forks without having the manufacturer approval to assure the safe operation and use of the forklift.
