29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts and flying chips: A. Bullard Area: On, or about, October 24, 2014, the shield provided around Bullard VTL 0013 was approximately 35 inches above the work platform on which the operator stood. The shield should have been at least 42 inches above the work platform. B. Tool Room: On, or about, November 19, 2014, the following Okuma horizontal lathes were not provided with chuck guards: M23, M24 and M25. C. Turning Cell: On, or about, November 14, 2014, the door interlock switches provided on Machine # 4391 and Machine #4392, two Mori Seiki SL-65 turning centers, had been bypassed. The door key had been removed from the access door and inserted into the key receiver. D. Football Field: 1. On, or about, January 8, 2015, the YCM M100 turning center was not provided with an interlock on the access door during the automatic cycle and the auger-type chip conveyor was not safeguarded either by the use of a constant pressure control, door interlock or physical barrier guards. 2. On, or about, November 14, 2014, the door interlock switches provided on Machines M18 and M19, two Mori Seiki Model SL-65 Turning Centers, had been bypassed. 3. On, or about, January 8, 2015, Machine M56, a Supermax V2110B turning center was not provided with an interlock on the access door during the automatic cycle and the auger-type chip conveyor was not safeguarded either by the use of a constant pressure control, door interlock or physical barrier guard. 4. On, or about, January 8, 2015, the door interlock switches provided on Machine M87 and Machine M88, two Puma 450 turning centers, had been bypassed. E. Back Tool Room Area: On, or about, November 19, 2014, the door interlock switch provided on Monarch VMC 175B milling center had been bypassed.
