29 CFR 1910.147(f)(3)(i): The employer does not utilize a procedure which affords the employees a level of protection equivalent to the protection provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance is performed by a crew, craft, department or other group: A recent example of the employer's violative conduct occurred on this occasion: a) 9000 River Rd, Delair, NJ, 08110-Extrusion Lines: Group lockout procedures were not utilized when employees performed checks on chains, rollers, conveyors; pad replacement, and retrieved metal from the hot saw puller and stretcher areas on the cooling table, on or about 1/25/2017. To abate this violation, the employer must establish and utilize a system to ensure that employees are utilizing a written energy control procedure for group lockout while conducting servicing and maintenance activities including cleaning and servicing operations on the extrusion lines. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, AND PURSUANT TO 29 CFR 1903.19, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, INCLUDING THE STEPS IT IS TAKING TO ENSURE COMPLIANCE WITH THIS STANDARD; ENSURE THAT ITS EMPLOYEES ARE UTILIZING GROUP LOCKOUT PROCEDURES WHILE PERFORMING SERVICING AND MAINTENANCE ACTIVITIES, INCLUDING CLEANING AND SERVICING THE EXTRUSION LINES. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19. ALUMINUM SHAPES, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR ESSENTIALLY SIMILAR STANDARD 29 CFR 1910.147(f)(3)(ii)(D),WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1059368, CITATION NUMBER 1, ITEM NUMBER 12, AND WAS AFFIRMED AS A FINAL ORDER ON 7/5/2016 WITH RESPECT TO A WORKPLACE LOCATED AT 9000 RIVER ROAD, DELAIR, NEW JERSEY, 08110
