29 CFR 1910.217(c)(3)(iv): The pull-out device shall protect the operator as specified in paragraph (c)(3)(i)(b) of this section, and shall include attachments for each of the operator's hands: On or about 14 February 2018, the employer failed to protect the employees from amputation while operating mechanical power press(es). Pullout device(s) used on mechanical power press(es) did not include wristlets for each of the operators' hands and were not adjusted to withdraw the hands from the point of operation if they were inadvertently placed in the die space. On of Press #43 one pullback cable was completely removed. Abatement documentation is not required for this item.
