29 CFR 1910.147(c)(4)(i): The employer does not ensure that procedures are utilized for the control of potentially hazardous energy when employees are engaged in activities covered by this section: A recent example of the employer's violative conduct occurred on this occasion: a) 9000 River Rd, Delair, NJ 08011-Extrusion Lines: Employees that performed checks on chains, rollers, conveyors; pad replacement, and retrieved metal from the hot saw puller and stretcher areas on the cooling table did not utilize lockout procedures. Procedures not followed included, but were not limited to, placing a lock on the electrical panel, locking out the natural gas line, locking out the airlines, and testing for residual energy, on or about 1/25/2017 & 2/8/2017. To abate this violation, the employer must establish and utilize a system to ensure that employees are utilizing the written procedures while performing servicing and maintenance activities, including cleaning and servicing operations on the extrusion lines. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, AND PURSUANT TO 29 CFR 1903.19, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, INCLUDING THE STEPS IT IS TAKING TO ENSURE THAT ITS EMPLOYEES ARE UTILIZING THE ENERGY CONTROL PROCEDURES WHILE PERFORMING SERVICING AND MAINTENANCE ACTIVITIES, INCLUDING CLEANING AND SERVICING THE EXTRUSION LINES. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19. ALUMINUM SHAPES, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR ESSENTIALLY SIMILAR STANDARD 29 CFR 1910.147(c)(4)(ii),WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1059368, CITATION NUMBER 1, ITEM NUMBER 10, AND WAS AFFIRMED AS A FINAL ORDER ON 7/5/2016 WITH RESPECT TO A WORKPLACE LOCATED AT 9000 RIVER ROAD, DELAIR, NEW JERSEY, 08110.
