29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) Warehouse, Building 14: Operator training provided only related to a Hyster 40 H4DXL propane sit down forklift. Operator training was not provided specific to a Crown WR Series electric walk behind forklift, a Crown WP Series 2300 electric motorized pallet jack, and four PHS West Ergo Express electric motorized flatbed carts.
