29 CFR 1915.152(b)(4): The employer failed to document an occupational hazard assessment which should have included: Occupations involved in the survey, the date in which the survey took place and the name of the person who performed the hazard assessment. Bldg. 1405, 1396: On or about December 12, 2024, and at times prior thereto, a hazard assessment had not been documented for the following operations: a) Welding operations b) Grinding operations c) ergonomics assessment d) ballast installation e) Band saw use Failure to conduct, document and communicate the required hazard assessment to the effected employees potentially puts them at risk for occupational injury.
