29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: a) Extrusion Line 7- On or about June 21, 2019, employer did not ensure that a stepdown transformer had a cover and was open with exposed live wires, exposing employees operating the machine to electrical shock hazards up to 480 volts. b) Extrusion Line 2 - On or about June 21,2019, A fitting was open on a control panel with exposed wiring up to 120V.
