29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service: a) Throughout facility: The employer did not ensure that employees operating a powered industrial truck in the facility, including, but not limited to, a Harrington Manufacturing Company lift, model 8F8000T serial number 3111, examined the truck prior to placing it into service. The horn and the back up alarm on the truck were inoperable, on or about 8/11/17. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
